Summary of Benefits
Vacation:
- Vacation hours for new full-time employees will start to accrue at the rate of 3.0769 hours per pay period, equaling 80 hours of vacation by the end of the first year of employment.
- Employees working less than 40 hours per week will receive a percentage of paid vacation based on the average number of hours or days worked.
- Full-time and part-time newly hired employees will be eligible to use accrued vacation benefits at the point in time when they have completed 90 days of employment.
- Employees working less than 20 hours per week are not eligible for vacation time.

Sick Time:
- Sick time accrual will be earned at 1 hour for every 30 hours worked.
- Exempt employees are presumed to work 40 hours in each workweek for purposes of sick time accrual.
- Full-time and part-time newly hired employees will be eligible to use accrued sick time at the point in time when they have completed 90 days of employment and have worked a minimum or 240 hours.
- Maximum carry over to subsequent years is set at 40 hours.

Holidays:
Six paid holidays per year - Memorial Day - Independence Day - Labor Day Thanksgiving - Christmas - New Year's Day Note: Employees are eligible for holiday pay after 30 days of service.
Medical Benefits/Eligibility:

- You are eligible for benefits if you work 30 or more hours per week. You may also enroll your eligible family members under certain plans you choose for yourself. Eligible family members include:
- Your legally married spouse.
- Your Registered Domestic Partner (RDP) and their children, where applicable by state law.
- Your children who are your natural children, stepchildren, adopted children, or children for whom you have legal custody (age restrictions may apply). Disabled children age 26 or older who meet certain criteria may continue on your health coverage.
- You must complete the enrollment process within 30 days of your date of hire.
- There are 3 medical coverage options to choose from.
- After one year, employees are eligible for an employer contribution of $200.00 per month towards dependents' coverage.
- Employees should check the summary of benefits from Regence BlueCross Blue Shield to identify in-plan providers in order to further reduce health care costs.
- You can call or go online for a list of doctors on the provider's list.
- Open enrollment is in May.
Dental Benefits/Eligibility:

- Employee is eligible 30 days after hire. Insurance is effective the first of the month following the 30th day of employment.
- You must complete the enrollment process within 30 days of your date of hire.
- There are 2 dental coverage options to choose from.
- You may go to any dentist of your choice. Employees should check the summary of benefits from Lincoln Financial for a list of in network dentists by plan choice.
- By enrolling in the dental plan you and your enrolled family members will have access to Lincoln Dental Connect free on-line dental health information Website.
- Dependent children may be covered up to age 26.
- Open enrollment is in May.

- Employee is eligible 30 days after hire. Insurance is effective the first of the month following the 30th day of employment.
- You must complete the enrollment process within 30 days of your date of hire.
- Benefits include well vision exam, prescription glasses, lenses, frames, contacts and more.
- Open enrolment is in May.
Flexible Spending Accounts:
Health Care Flexible Spending Account
- You may contribute up to $2,650 to cover eligible health care expenses incurred by you, your spouse, and your children up to age 26.
- Eligible expenses include: Coinsurance, copays, deductibles, dental treatment, vision care and prescriptions.
- Unused health care funds over $500 will not be returned to you or carried over to the following year.
Dependent Care Flexible Spending Account
- You may contribute up to $5,000 annually (per family) to cover eligible dependent care expenses.
- Eligible expenses include: Care of a dependent child under the age of 13 by babysitters, nursery schools, pre-school or daycare centers; Care of a household member who is physically or mentally incapable of caring for themselves and qualifies as your federal tax dependent.
- Unused dependent care funds will not be returned to you or carried over to the following year.
Short Term Disability Insurance:
- STD insurance provides weekly income to disabled/ ill employees.
- 60% of weekly salary up to $250 per week.
- Benefits begin on the 15th day for a qualifying Accident/Illness.
Group Term Life Insurance:

- Company covers each employee with a $10,000 Term Life and a $10,000 Accidental Death and Dismemberment benefit for as long as they are with the company.
Group Long Term Disability Insurance (Is a voluntary/optional supplemental Long Term Disability insurance)

- Employee is eligible 30 days after hire. Insurance is effective the first of the month following the 30th day of employment.
- In order to be eligible, the employee must be enrolled in Regence Blue Cross Blue Shield insurance through GloryBee.
- Maximum benefit payout is 50% of monthly salary up to $1,000 per month.
- Employee must be out 180 continuous days to be eligible.
Optional: MetLife (Is a voluntary/optional supplemental Term Life insurance)
- Employees working at least 30 hours per week.
- Open enrollment is May.
Optional: Pet Insurance (Is a voluntary/optional supplemental Pet insurance) - www.PetsNationwide.com (visit for quotes and My Pet Protection plans are available exclusively through your employer.
Retirement:
401 (K) Plan and/or 401(K) Roth Option
- Full and part time employees are eligible after 12 months plus 1,000 hours worked. If the employee does not meet the 1,000 hour requirement in the first anniversary year, the requirement switches to the plan year. Enrollment is in the beginning of the quarter following this period.
- Administered through Voya Investment Management.
- You may contribute up to 100% of your salary to your 401 (K) annually. $175,000 maximum per year unless you qualify for make-up contributions.
- GloryBee will match up to 4%, anything over 4% will not be matched.
Health Incentive Program:
- Healthy Living Rebate #1 - Employees can choose to participate in either a gym membership, weight loss and or Sit Stand Station (proof of payoff from the Accounting Department prior to reimbursement) program. A one time per calendar year reimbursement of $75.00 with proof of payment and supporting membership documentation.
- Healthy Living Rebate #2 - Employees who are currently participating in Healthy Living Rebate #1 and have spent $200.00 towards the combined memberships, can choose to add the other option (either Gym membership or Weight Loss program) one time per calendar year and be reimbursed an additional $50.00 with proof of payment and supporting membership documentation.
- Walk or ride your bike to work: You can be reimbursed $25 quarterly for choosing this healthy living choice.
- GloryBee offers partial reimbursement for walking and running events.
- Tobacco Free Incentive - Reimbursement of up to $500 for completing a tobacco-free program.

- Services are available to you and your qualified dependents at no charge.
- Each qualified dependent has 4 free visits per topic per year.
- It is confidential; you make your own appointment.
- Company receives no information on voluntary services.
- May be used for work related issues. When referred by GloryBee, the counselor will provide progress reports to the company.
Bushnell University
- Assistance and Guidance are available through the EAP.
- Services are available to you and anyone in your household 16 years or older at no charge.
- Each qualified dependent has 4 free visits per topic per year.
- It is confidential; you make your own appointment.
- Company receives no information on voluntary services.
